What combines nausea, vomiting and eye swelling?

 

The recent case in the ACG case reports journal highlights an exciting case of non-Hodkin lymphoma-gastric Extranodal NK/T-cell (ENKTL) lymphoma presenting as a gastric ulcer with orbital involvement!

The patient presented with nausea, vomiting, weight loss, and intermittent swelling of the left eye.
While labs were positive for lymphopenia, the EGD revealed multifocal ulcers of the stomach, while biopsies of the ulcer and left orbital fat were positive for ENKTL.

The patient was initiated on steroids and PPI; however, she developed severe GI bleeding; due to the diffuse nature of the ulcers, EGD was not able to control the bleeding. The patient was started on comfort care and died from hemorrhagic shock and multiorgan failure.
